Aggravation of Multiple Sclerosis after L4-5 Posterolateral Fusion with CCD instrument

Abstract
As a treatment modality in spondylolisthesis. Posterolateral fusion with CCD (compact Cotrel Dubousset) instrument is a widely used method for its advantages such as less postoperative complication and better result than any other methods.
Aggravation of silent multiple sclerosis involving thoracic segments was observed in 48 years old female patient following posterolateral spine fusion with CCD instrument from the fourth to fifth lumbar vertebra. The clinical symptoms were
developed
shortly after the operations. At 1 hour after the surgery. The patient suddenly became paraplegic ; the sensory loss was present below the T7. And then sensory loss was aggravated below the T3. There was an impairment of sphincter control of the
bladder
and rectum. The deep tendon reflexes were diminished. The cerebrospinal fluid was not abnormal finding. In computed tomogram and MR imaging of spine thal were taken the day after tomorrow. Abnormal instrument insertion and abnormal signal
intensities
were not seen. On follow up MR imaging that was taken 8 weeks after surgery, several high signal intensities along the cervical and thoracic spinal cord were observed in the T2 weighted imaging. On the last follow up. The motor power and
sphincter
control were improved but the sensory loss was not change.
